    • We're located in Yorktown, Indiana, surrounded by natural beauty including Morrow's Meadow and the White River Greenway Trail. This peaceful setting provides a quiet, residential feel while keeping families connected to nearby amenities and healthcare services.

    • Heritage Assisted Living of Yorktown offers both assisted living services for seniors requiring minimal daily support and specialized memory care for residents with dementia-related conditions. We also provide short-term respite stays and coordinate with hospice services when needed. Our mixed population ranges from age 55+ through centenarians, and we're equipped to support residents as their needs change over time.

    • Yes, Heritage Assisted Living of Yorktown accepts Medicaid waiver, making quality care accessible and affordable for families who need financial assistance. We're committed to ensuring that cost doesn't prevent your loved one from receiving compassionate, personalized care in a beautiful, dignified setting.

Care Services & Approach

    • Our philosophy centers on dignity, independence, and meeting residents where they are. We believe in maximizing each resident's independence while providing personalized support that respects their life story, preferences, and capabilities. Our staff takes time to understand each resident as an individual, adapting our approach to honor what matters most to them.

    • Our team members receive specialized training in both assisted living and memory care, including dementia care best practices, medication management, and person-centered care approaches. We maintain staffing ratios that allow for personal attention and meaningful relationships. Staff members are accessible to families - we believe in open communication and provide personal contact information to ensure families can reach us when needed.

    • This means we adapt our care to each resident's current abilities, preferences, and needs rather than imposing a one-size-fits-all approach. If a resident prefers to wake up later, we accommodate that. If they have specific routines or preferences from their lifetime, we honor those. We focus on what residents can do rather than what they can't, supporting their strengths while providing assistance where needed.

    • Our knowledgeable care team coordinates closely with residents' physicians, specialists, and healthcare professionals to ensure comprehensive care. We manage medication schedules, attend to healthcare appointments, and maintain open communication with medical providers to support each resident's overall wellness.

    • Yes, our dual care model allows residents to remain in our community even as their needs change. Whether transitioning from assisted living to memory care or requiring additional support over time, we work to keep residents in the familiar, comfortable environment they've come to call home.

Assisted Living

    • Our assisted living services include support with activities of daily living (ADLs) such as bathing, dressing, grooming, and personal hygiene. We provide medication management, assistance with mobility, and support with daily routines - all tailored to each resident's specific needs. The goal is to help residents maintain as much independence as possible while ensuring their safety and wellbeing.

    • Very independent! We encourage residents to do as much as they can for themselves, providing support only where needed. Residents maintain control over their daily schedules, social activities, and personal choices. Our role is to enhance their independence by removing obstacles and providing assistance that allows them to continue living life on their terms.

    • We regularly assess each resident's needs and adjust care plans accordingly. If additional support is needed, we modify services to meet those changing needs. Our team communicates proactively with families about any changes, ensuring everyone is informed and involved in care decisions.

Memory Care

    • Our memory care program goes beyond physical care to focus on emotional wellbeing, comfort, and respect for both residents and families. We provide a secure Memory Care Neighborhood designed specifically for residents with Alzheimer's and dementia, featuring safety systems, way-finding cues, and specialized programming. Our staff receives ongoing training in dementia care best practices and understands how to communicate effectively with memory care residents.

    • Yes, our Memory Care Neighborhood features a secured environment designed to keep residents safe while allowing them freedom to move about comfortably. We use discreet security measures that don't feel institutional, and our outdoor spaces are safely enclosed so residents can enjoy fresh air and nature without risk.

    • We offer therapeutic activities designed to engage memory care residents at their ability level. This includes sensory experiences, music therapy, reminiscence activities, gentle physical movement, and structured routines that provide comfort and security. Activities focus on maintaining dignity and creating meaningful moments rather than highlighting what residents can no longer do.

    • Our staff is trained to understand that behaviors often communicate unmet needs. We respond with patience, compassion, and problem-solving rather than confrontation. We work to identify triggers, adjust the environment, and find personalized approaches that reduce anxiety and promote comfort for each resident.

    • Absolutely. Family involvement is crucial in memory care, and we welcome visits during our visiting hours. We encourage families to participate in activities, join for meals, and maintain their connection with their loved one. Our staff can provide guidance on visit approaches that work best for your loved one's current stage.

Daily Life & Activities

    • Each day includes opportunities for social engagement, physical activity, creative expression, and personal time. We offer structured activities throughout the day, but residents choose their own level of participation. Meals are served in our dining areas where residents gather as a community. Staff members are available 24/7 to provide care and support whenever needed.

    • Our Life Enrichment Programming focuses on four dimensions of wellness, keeping residents physically active, mentally engaged, socially connected, and emotionally fulfilled. Activities are planned with resident input - we want to know what interests you! Programming includes everything from crafts and games to exercise classes, music, outings, and intergenerational activities that connect residents with the broader community.

    • Residents have an active voice in influencing community cuisine. We serve nutritious, home-style meals in communal dining areas that encourage social interaction. Special dietary needs are accommodated, and we work to incorporate resident favorites and requests into our menus.

    • We believe connecting generations enriches everyone's lives. We coordinate activities that bring children and young people into our community for shared experiences like holiday celebrations, reading programs, arts and crafts, and musical performances. These connections help residents stay engaged with the broader community and provide meaningful interactions that bring joy to all ages.

    • Assisted living residents who are capable can come and go freely, though we ask that they inform staff for safety purposes. Memory care residents have movement within secured areas for their safety. We encourage families to take residents on outings and maintain connections to the community whenever appropriate.

Family Involvement & Communication

    • Family involvement is not just welcomed - it's essential to our care model! Families are encouraged to visit regularly, participate in care planning meetings, join activities and meals, and maintain open communication with staff. We consider families true partners in caring for your loved one.

    • We believe in proactive communication. Staff members provide their personal contact information so families can reach us directly with questions or concerns. We provide regular updates on your loved one's wellbeing, notify families promptly of any changes or concerns, and welcome ongoing dialogue about care. We use multiple communication channels including phone calls, emails, and in-person conversations.

    • We maintain reasonable visiting hours that allow families flexibility to visit when it works for their schedules. Specific hours can be discussed during your tour. We want families to be present and involved in their loved one's life at Heritage.

Many families use a combination of private pay and Medicaid, transitioning to Medicaid waiver when private resources are exhausted. We're happy to discuss Medicaid eligibility and the application process.

    • We support residents through this transition. If a resident's financial situation changes, we work with families to apply for Medicaid waiver and continue providing the same quality care they've been receiving. Our goal is to allow your loved one to remain in the community they've come to call home.

    • We aim to keep additional costs minimal. Most services are included in your monthly rate. Any additional services or specialized care needs will be discussed upfront so you understand the full cost of care.
